Welcome to the new patient portal by ModMed

Access and update your health information, request refills, view lab and imaging results, conduct telehealth visits — all safely and securely.

If you have already have an account, you can enter the patient portal

Steps to activate your new account

1.

COUG will activate your portal by utilizing your mobile phone number.

2.

COUG will send you an e-mail which contains the portal link

3.

Click the link in the e-mail. This will take you to a screen to enter your first name, last name, date of birth and e-mail address.

Once your first name, last name, date of birth and e-mail address are verified, you will enter your mobile phone number. Use the same mobile phone number that is in your patient record with COUG.

5.

Now, select “Send Code”.

Trouble receiving your password reset link or needing other portal assistance

THIS FORM IS FOR PATIENT PORTAL ISSUES ONLY.  For any other needs such as prescription refills, appointments or medical concerns, please contact the office at (614) 396-2684. In the event of a medical emergency, please dial 911 or go to the nearest emergency room.

While waiting for our response, you may confirm the following to ensure success with the portal:

  • You are attempting to log into the correct practice portal for Central Ohio Urology Group
  • You are connected to the internet
  • You have attempted a password reset from the portal
