Message to Our Patients and Employees Regarding Recent Data Incident

Central Ohio Urology Group is informing individuals about a data security incident. An unauthorized individual posted files and documents to an online drive accessible on the Internet that appear to have been maintained on Central Ohio Urology's internal file server. The files and documents included personal information about patients, employees, and in some cases people that paid for medical services.

We are working with a computer forensics firm to investigate this incident and are updating system protections to help prevent this type of incident from recurring in the future. Based on the findings of our investigation to date, we are providing the following information and resources:

  • A detailed Notice that explains what happened, describes the actions we've taken, and provides information and resources to anyone who may have been affected.
  • Frequently Asked Questions that provide additional information about the incident.
  • An Information About Identity Theft reference guide with information about how you can protect your identity, including information about credit monitoring services.
  • Access to a call center with operators standing by to address questions and concerns about this incident. You can reach this call center by dialing 1-844-749-5108.
